Valentine questionaire

Fill in the questionaire and give Patsy some insight about your loved one. She will draw inspiration from your responses to design and create, with exotic papers, or whatever else comes to mind, a very special personalized Valentine card for your sweetheart ...

The completed questionaire will be integrated within the card, in a secret pocket, a special touch sure to trigger his/her curiosity ...

Small but helpful instructions to follow:

1. Print out the questionaire.
2. Take some time to visualize your love.
3. Fill in the questionaire.
4. Add a photo, poem, song, quote (or pieces of) ... Anything you think I could use as inspiration for a card for this person could be useful.
5. Mail the questionaire to Patsy, with your $50 CAN* cheque (payable to PAR PATSY)
